Before any treatment can take place it is a legal requirement to have veterinary consent via a signed consent form – see below. This allows me to communicate with your vet and inform them of my findings to make sure your animal gets the best treatment and therefore the best results.
Once veterinary consent has been obtained and your animal has had their initial consult I will report back to your vet with my findings and plan of action. I will also keep them up to date with their progress throughout treatment as if at any point your animals' condition changes this will allow easy referral back to your vet for further investigation and treatment.
To gain veterinary consent and allow treatment to commence please complete the form below and get your vet to sign it. You can then email it to me on firstname.lastname@example.org or hand it to me at your initial consult.
